I think because I have had to hack the comment posting for to get one of my old spam blockers to work, Spam Karma is now getting confused and thinks that comments are being blocked when they are infact being allowed through.

So I now get two emails, one to say its been blocked and the other to say that it has been authorised. Yay!

Spam Karma can do this automatically for you I believe!
